In addition to providing excellent, high-quality specialized care for the community, the outpatient programs at Dominican Health and Rehabilitation Services provide a seamless transition of care from inpatient or Home Health programs. The emphasis is on a team approach, centered around the goals of the patient and their family.
Several outpatient programs are available, including:
Center for Lifestyle Management
Services include cardiac risk reduction and rehabilitation, pulmonary rehabilitation, diabetes management, medical nutrition therapy and prevention/wellness programs. Dominican’s cardiac risk reduction, pulmonary rehabilitation and diabetes management programs are all nationally certified.
Hand & Arm Rehabilitation
Services include evaluation and therapeutic management for patients who have injury or illness affecting hand or arm function, custom splint or brace fabrication, and individualized therapeutic exercise programs. Dominican’s Hand & Arm Rehabilitation program is staffed by certified hand therapists.
Neurological Therapy Services
Services include occupational, physical, speech and aquatic therapies for individuals with neurological impairment; Parkinson’s and balance therapy groups; vestibular rehabilitation; post-amputation rehabilitation; fall prevention program; mobility services and Lymphedema therapy. Dominican Hospital is a Certified Stroke Center, and has staff with Neurological Clinical Specialist, NDT and Neuro-IFRAH certifications, as well as LANA board certified therapists and certified assistive technology providers.
