Patient Services
Skilled nursing care: medical management of chronic conditions; pain and symptom management; diabetic care; intravenous (IV) nursing; post-surgery follow-up; new mom/baby visits; patient/caregiver education; wound care.
Physical therapy: exercises to help regain function, strength, balance and endurance; gait training to improve walking; pain management modalities; instruction in use of wheelchairs, walking aids and artificial limbs; home safety evaluation.
Occupational therapy: assessment and training to achieve independence in self-care, including the use of adaptive equipment as needed; conservation of energy to increase endurance.
Speech and language therapy: help in regaining speech, language, writing and other information processing skills; rehabilitation services for swallowing difficulties.
Supplementary patient and caregiver support services
These services are available only when skilled nursing services are in place:
- Home health aide: supervised assistance with personal care; patient/caregiver instruction in maximizing patient comfort and safety.
- Medical social work: short-term counseling and support services; long-term planning or placement assistance; connection of patients/caregivers with appropriate community resources.
- Nutrition and dietary consultations.
