Services provided by Dominican's palliative care team include:
- Pain and symptom management, especially for complex cases where relief is difficult to achieve.
- Emotional and spiritual support.
- Information on Advance Care Planning ot help make informed decisions on life-sustaining and life-extending measures, their benefits and burdens.
- Assistance with arrangements for post-hospital care.
- Help with making end-of-life decisions, or making or resolving bioethical dilemmas.
- Support for loved ones and caregivers.
- Coordination of care and communication among patient, family, physicians and other care providers.
- Referral to community and outpatient resources such as rehabilitation, home health and hospice.
