For Our New Patients: Where to Go and What to Bring
Before You Arrive:
Your doctor has arranged with the hospital for your admission. We will call you at home a few days beforehand to obtain registration information, and answer your questions. You will also be requested to have certain tests performed a few days before you are admitted.
Well before your arrival at the hospital, you will also want to make arrangements through your physician for any self-donated blood transfusions or other products.
This is also the time to decide on the levels of care you desire, and have considered and signed Durable Powers of Attorney or other Advance Directives. Dominican offers seminars on Advance Directives.
Call 462-7709 for the next dates. Copies of the documents are always available in the Hospital Gift Shop. To download copies of the documents in English and Spanish, click on either the PDF or Word document below.
