• Evaluation and treatment of episodic medical conditions
• Identification of medical homes for those patients with chronic needs
• Indentification of social services and resources in the community

Cost:
A fee of $15 will be requested at the time of visit.

Schedule:

Clinic hours are 9am to 3pm except on holidays or as posted
